I don't get an option to set up a chart of accounts in setup.pl.
Is that supposed to be done manually?

Ok, here is how it currently works:

In the setup.pl, you create the database.  Then it gives you an option to create the chart of account (selecting the country and chart) or skipping this, then going to the user creation screen.  

We now have an ability to import CSV COA's and so we allow skipping the step because that allows people to set up their own COA later.
