I was mistaken in thinking that there was a problem with the above
mentioned files.  They are fine (other than changing formatting to my
preferences).  Turns out it is a CUPS issue.  I had to give the apache
user rights to print on the default printer.  Now they print out
without any troubles at all.  Thanks all for the help!
