The problem is an omission/error. Easy to fix. Just download the latest form.pm from the trunk directory from the source code respository. I don't know why/how this could have happened. I ran it up the flag pole to the maintainers, but I guess they haven't replaced the downloadable tar ball yet.
You simply need to add the follow line (just preceding the database commit line):
@{$self->{all_years}} = @{$dbh->selectcol_arrayref($query)};
to the all_years subroutine in form.pm to enable choose-able periods when doing reports.
