I would say it is likely in the hundreds.
My most impressive implementation is that of Orondo Cider Works (an
orchard/cider manufacturing/retail business with probably 1k-2k
invoices per week). Up to three users are online at any point, but
this takes every invoice from the cash register stations. I have been
working to ensure that they are able to use hand-helds efficiently,
etc.
Most of my customers are service providers though, and they implement
it for their customers.
Our core development team consists of 6 contributors, and we have had
contributions from probably another dozen or so people.

One additional question -- do you, or anyone else, have a sense for how
many companies are successfully using LedgerSMB? Or, even better,
examples of companies that are using it?
