Thank you for the suggestion.
I tried two approaches and in both cases the base schema failed to load.
1. I inserted ‘use Carp::Always’ at line 30 in setup.pm and ran http://locallhost/ledgersmb/setup.pl in the browser as before. Generated the same error.
The apache error log for the sequence after I selected “Yes” to create the database lsmbdb141 (not sure why stack trace info does not appear):
