that a row in menu_attribute table with attribute = 'menu' and
value = '1' caused the problem.

Using stored procedure menu_insert() makes it easy to insert a menu item. Looks like store procedure menu_children() is operating correctly, but stored procedure menu_generate() seems to have a bug. After adding a menu item with lineage: Goods and Services - Reports (node ID = 85), the rest of menu is no longer correct. Main menu items stop at Goods and Services (i.e., everything is now under "Goods and Services"); "Projects" should be a main menu item, but is displayed under "Goods and Services".
Does anyone have some experience with adding a menu item they can share?
