Installation issues Mac OS X 10.6.8 lsmb 1.3.15

I have been trying to install lsmb 1.3.15 on my Mac for a while.  I
haven't taken a stab at it for a while (hence the old version), when I
try to login to:

http://localhost/ledgersmb/setup.pl

I get:

Access Denied:  Bad username/Password

Acknowledging that gives me:

Internal Server Error

with all the default details (my server hasn't been setup to go beyond
the defaults).

In the error.log I get:
