Think I may have finally found a valid test case that seems to cause the 
problem.  For me, I have a tax account called GST Collected.  With a default 
dataset, I don't see any problem using the built in Chart of Accounts GST, it 
is when I have created a Tax account called "GST Collected" and set a vendor 
to have that Tax, as well as a service to be using that tax account, and then 
create and invoice, that the Tax does not register.  I'm having to run out 
for a bit, but I can send back more details and an example broken dataset 
soon now.

Maybe that helps to reproduce.

Problem is, all my old accounts are set up this way...
