Report Headers

Hello!

I recently upgraded my companies from 1.2.26 to 1.3.16.  Unfortunately, the company name and address information doesn't seem to have made it in the transfer.  When I print reports, the company name and address is no longer at the top of the report.  Where would I go to enter this information?
