can't log in to setup.pl

I installed ledgersmb 1.3.4 on my centos 6 based server with apache 2.2
and postgresql 8.4. I followed the install guide at
ledgersmb.org/news/ubuntu-installation-tutorial-ledgersmb-13-series.
When I get to the part to login at localhost/ledgersmb/setup.pl I put
the postgress password I created and it gives me a bad username password
error.
the error log reads as follows.

After much searching I am unable to find an answer to this issue. Where
should I go from here?
