Only a superuser of PostgreSQL.

Same for user ledgersmb I guess?  Does user ledgersmb _have_ to have password ledgersmb?  Also, can I create a ledgersmb database called something else, e.g. Dodgy Brothers' Rorts?

For 1.3 typically you will create a db and initial user using the postgres user, and other users will be created by that one.

You don't need a special system account for ledgersmb.
